United PPO Breast Reduction Insurance Coverage


Unfortunately, most United Healthcare PPO insurance plans do not cover breast reduction surgery. In fact, they frequently have something called a “plan exclusion,” meaning that breast reduction surgery is specifically excluded from their coverage plans.

A few United PPO plans may cover breast reductions, for the plans that do, United PPO considers a breast reduction to be a “functional operation,” meaning that the purpose of a breast reduction is to improve the symptoms caused by large breasts such as rashes, shoulder pain, and neck pain. Breast reductions can also treat breast cancer or be used during gender confirming surgery, but these are undergo a different authorization than other breast reduction procedures and in fact these are usually covered by United PPO plans.

If it is a covered benefit, then the authorization process is the same as every other surgical procedure.

How much does a breast reduction cost if it is covered by insurance?

If Blue Shield covers your breast reduction, you are typically responsible for your in-network deductible and your in-network co-payment. Depending on the facility that is used for your surgery, Dr. Hirsch may be able to accept your insurance.
 

What is the difference between a breast lift and a breast reduction?

A breast reduction reduces the size of the breast and lifts/reshapes the breasts. A breast lift does not usually make a significant difference in size and only lifts/reshapes the breasts. A breast reduction can be covered by your Blue Shield PPO insurance, while a breast lift is not usually covered by insurance.
